RESUMO

The World Health Assembly in May 2021 was a watershed moment in oral health, with the landmark resolution that designated oral diseases as non-communicable diseases (NCDs). This was strongly supported by a host of other NCDs in recognition of the common risk factor principle and acknowledgement of the fact that oral diseases do not occur in isolation from other NCDs, but are commonly associated with cardiovascular disease, diabetes/obesity, respiratory diseases, metabolic syndrome, a range of other inflammatory disorders and cancers. Regular monitoring and early detection would potentially intercept these NCDs and this could form a central plank of a revamped holistic 'health'- as opposed to 'disease'-oriented health care system.Consultation with patients and dentists reveals strong support for maintaining regular recall intervals, which maintains trust and optimises motivation and compliance. In-person visits could be minimised by using technology, such as remote consultations and longitudinal monitoring systems, making it adaptable to different health care settings and equitable, affordable, cost-effective and sustainable.A new paradigm with dentists as oral health professionals, and the mainstreaming of oral health and population-level prevention, means the future of health care can be guided by integration and workforce modification producing a surveillance-based, early interceptive, preventive model of care.

RESUMO

The modern healthcare landscape is overwhelmed by data derived from heterogeneous IoT data sources and Electronic Health Record (EHR) systems. Based on the advancements in data science and Machine Learning (ML), an improved ability to integrate and process the so-called primary and secondary data fosters the provision of real-time and personalized decisions. In that direction, an innovative mechanism for processing and integrating health-related data is introduced in this article. It describes the details of the mechanism and its internal subcomponents and workflows, together with the results from its utilization, validation, and evaluation in a real-world scenario. It also highlights the potential derived from the integration of primary and secondary data into Holistic Health Records (HHRs) and from the utilization of advanced ML-based and Semantic Web techniques to improve the quality, reliability, and interoperability of the examined data. The viability of this approach is evaluated through heterogeneous healthcare datasets pertaining to personalized risk identification and monitoring related to pancreatic cancer. The key outcomes and innovations of this mechanism are the introduction of the HHRs, which facilitate the capturing of all health determinants in a harmonized way, and a holistic data ingestion mechanism for advanced data processing and analysis.

Introduction Manatu Hauora (Aotearoa New Zealand (NZ) Ministry of Health) recognises that respecting and listening to Pacific peoples' knowledge and skills in caring for their communities' wellbeing is a priority, and that novel approaches to care, particularly for chronic health conditions, are necessary. Allied health professionals have the potential to play important roles in primary care design and delivery. Pacific Trust Otago (PTO) provides a weekly seniors' group gathering that has evolved over the years and incorporates exercise, health information and cultural activities. Aim This study aims to explore what Pacific seniors are learning about their health and wellbeing, and what factors contribute to sustained engagement with this weekly group activity. Methods The research team conducted a qualitative study using the Kakala Research Framework and the pan-Pacific Talanoa Research Methodology to gather narratives in a relational and comfortable group space. Participants were recruited from the seniors group gathering. Group Talanoa were used to collect data, which were digitally voice-recorded, transcribed, translated, and de-identified. Ethical approval was granted by the University of Otago School of Physiotherapy Ethics Committee. Results Pacific seniors valued how these gathering The seniors recognised how interconnected and intertwined culture, spirituality, and family were and how this contributed to their sense of individual and collective health and wellbeing. The study highlights the need to consider health beyond the individual to the collective, embracing indigenous perspectives, and authentically nurturing relationships with Pacific health providers. The study recommends primary care funders are supported to understand what is important to Pacific people and partner with Pacific health providers to deliver care in ways that align with indigenous models of care. Discussion A weekly group gathering for Pacific seniors run by Allied Health professionals provides a welcoming, safe, and culturally meaningful environment where seniors can connect, share, and grow in health and wellbeing together. This study highlights the importance of adopting informed and inclusive approaches to promoting and addressing holistic health for Pacific people, especially in light of ongoing health reforms in Aotearoa NZ.

Poland has a unique history of traditional Chinese medicine (TCM) dating back to the 17th century when Polish missionary Michael (Michal) Boym was a pioneer in the field. In the 20th century, his successor, Professor Zbigniew Garnuszewski, reintroduced acupuncture to medical practice in Poland. However, other methods of TCM and its holistic approach to patient care have not found their place in modern medicine in Poland. At present, the legal status of TCM in Poland remains unregulated, with TCM included in the broad spectrum of complementary and alternative medicine (CAM) practices. Few reports are available on the use of TCM methods among the Polish population. Integrative medicine combines conventional medicine with evidence-based CAM interventions and considers all aspects of a patient's health, including physical, emotional, mental, social, and environmental factors. An integrative healthcare model that incorporates TCM modalities and lifestyle recommendations as well as a whole person approach may provide a more sustainable solution for the constantly underfinanced Polish healthcare system, which faces challenges of multimorbidity in an aging society and limited access to care. The coronavirus disease 2019 pandemic, war in Ukraine, and ongoing climate crisis have underscored the need to strengthen the resilience of the Polish healthcare system and search for new solutions. A model of care that blends the best of biomedicine and TCM healing approaches may be a better option for both patients and the healthcare system in Poland. People living with treatable-but-not-curable (TbnC) cancer encounter cancer-related needs. While the NHS long-term plan commits to offering a Holistic Needs Assessment (HNA) and care plan to all people diagnosed with cancer, the content, delivery and timing of this intervention differs across practice. Understanding how people make sense of their cancer experience can support personalised care. A conceptual framework based on personal narratives of living with and beyond cancer (across different cancer types and all stages of the disease trajectory), identified three interlinked themes: Adversity, Restoration and Compatibility, resulting in the ARC framework.Our aim was to use the ARC framework to underpin the HNA to improve the experience of personalised care and support planning for people living with TbnC cancer. We used clinical work experience to operationalise the ARC framework and develop the intervention, called the ARC HNA, and service-level structure, called the ARC clinic. We sought expert input on the proposed content and structure from patients and clinicians through involvement and engagement activities. Delivered alongside standard care, the ARC HNA was piloted with patients on the TbnC cancer (myeloma and metastatic breast, prostate or lung) pathway, who were 6-24 months into their treatment. Iterations were made to the content, delivery and timing of the intervention based on user feedback.Fifty-one patients received the intervention. An average of 12 new concerns were identified per patient, and 96% of patients achieved at least one of their goals. Patients valued the space for reflection and follow-up, and clinicians valued the collaborative approach to meeting patients' supportive care needs. Compared with routine initial HNA and care plan completion rates of 13%, ARC clinic achieved 90% with all care plans shared with general practitioners. The ARC clinic adopts a novel and proactive approach to delivering HNAs and care plans in a meaningful and personalised way.

Background: Public health interventions that target children's physical, mental, and emotional health will enhance their ability to learn and grow. Although more complex, school initiatives that address multiple ecological levels and take a holistic view may be more effective and likely to lead to lasting change. Aims: This article presents the framework of Commit to Be Fit (C2BF) as an example of how schools can integrate multi-level and holistic approaches for health. This innovative school-based intervention includes activities addressing individual, home, school, and community to create a culture of wellness. We describe the implementation of C2BF and its basis in ecological models and give examples of activities across three components: cafeteria, classroom, and community. We discuss challenges and note that leadership engagement and alignment were critical elements for C2BF's success thus far. Discussion: C2BF uses a school-based multi-level approach to creating a culture of wellness and holistic health for students, teachers, and community members. C2BF is unique compared to other school-based programming and includes activities that address all eight domains posited for program sustainability within public health. Built to be flexible and adaptive, C2BF was able to successfully pivot during the COVID pandemic and also follow new science. Conclusion: C2BF and other multi-level holistic approaches are more likely to achieve long-term change by utilizing strategies across the multiple levels of the ecological model to improve health and wellbeing.

Objetivo: Analisar casos de suicídio no Brasil de 2010 a 2021 e as características sociodemográficas deste fenômeno, incluindo os primeiros dois anos da pandemia de COVID-19, para avaliar a existência de relação dos aumentos de casos de suicídio no Brasil com esta doença. Métodos: Estudo ecológico de linhas temporais, com 140.339 casos analisados a partir das bases de dados do Sistema de Informações sobre Mortalidade (SIM). Taxas e regressões estatísticas foram aplicadas no software STATA®. Resultados: Observou-se maiores taxas de suicídio em homens, solteiros, e residentes nas regiões Sul e Centro-Oeste. A faixa etária de 10 a 19 anos teve crescimento de 1,7 casos x 100 mil habitantes/ano. A análise por região revelou variações significativas nas taxas, sugerindo influência de fatores contextuais. Embora a pandemia tenha gerado impactos na saúde mental, não foi possível estabelecer relação direta entre a COVID-19 e o aumento das taxas de suicídio. Conclusão: Esses resultados reforçam a necessidade de políticas públicas e intervenções preventivas, especialmente para grupos vulneráveis, como os jovens. Abordagem multifatorial é sugerida, considerando fatores socioeconômicos, acesso a serviços de saúde mental e redução do estigma associado com doenças mentais. Em suma, este estudo contribui para a compreensão das tendências temporais e características demográficas dos suicídios no Brasil, ressaltando a importância de investigações longitudinais adicionais para melhor compreensão desse fenômeno complexo. Espera-se que essas evidências fortaleçam as políticas de saúde mental e promovam estratégias mais eficazes de prevenção do suicídio.


Objective: To analyze suicide cases in Brazil from 2010 to 2021 and the sociodemographic characteristics of this phenomenon, including the first two years of the COVID-19 pandemic, to assess whether there was a relationship between the increase in suicide cases in Brazil and this illness. Methods: Ecological timeline study, with 140,339 cases analyzed from the Mortality Information System (SIM) databases. Rates and statistical regressions were performed using STATA® software. Results: Higher suicide rates in men, singles, and residents of the South and Midwest regions were observed. The age group of 10­19 years had an increase of 1.7 cases x 100 thousand inhabitants/year. Analysis by region revealed significant variations in rates, suggesting the influence of contextual factors. Although the pandemic affected mental health, it was not possible to establish a direct relationship between COVID-19 and increased suicide rates. Conclusion: These results reinforce the need for public policies and preventive interventions, especially for vulnerable groups such as young people. A multifactorial approach is suggested that considers socioeconomic factors, access to mental health services, and reduced stigma associated with mental illness. In short, this study contributes to understanding the temporal trends and demographic characteristics of suicides in Brazil, highlighting the importance of further longitudinal investigations to better understand this complex phenomenon. Hopefully, this evidence will strengthen mental health policies and promote more effective suicide prevention strategies.

Las enfermedades neurodegenerativas se caracterizan por un deterioro neuronal progresivo e irreversible. Aunque constituyen un grupo de patologías muy heterogéneo, la mayoría comparten un origen multifactorial y una estrecha relación con el envejecimiento. El aumento de la esperanza de vida a nivel mundial ha convertido a estas patologías en una amenaza para los sistemas sanitarios y de cuidados, pues su prevalencia crece de forma paralela al envejecimiento de la población. Destacan entre ellas, por su elevada prevalencia o por el impacto sanitario que tienen, las enfermedades de Alzheimer, Parkinson y Huntington; y la esclerosis lateral amiotrófica. El conocimiento sobre el origen de estas enfermedades es generalmente muy limitado, lo que condiciona la extrema dificultad de su manejo clínico. No existen métodos fiables para su diagnóstico temprano, por lo que su detección se suele producir cuando el deterioro clínico es ya irreversible. Por otra parte, los tratamientos disponibles hoy en día son meramente paliativos, ya que el desconocimiento sobre las causas primarias de la enfermedad y su diagnóstico tardío impiden el desarrollo de tratamientos capaces de detener o revertir el deterioro estructural y funcional. En los últimos años se han identificado varios fenómenos patológicos que podrían constituir las verdaderas causas primigenias de estas enfermedades, destacando el estrés oxidativo, las alteraciones de la proteostasis y la neuroinflamación: en las conexiones entre ellos y en su capacidad para retroalimentarse podrían estar algunas de las respuestas necesarias para avanzar hacia un abordaje clínico efectivo de los procesos neurodegenerativos.(AU)


Neurodegenerative diseases are defined by a progressive and irreversible impairment of neurons. Despite being a very heterogeneous group of pathologies, they share some common features, such as their multifactorial origin and their close correlation with ageing. As worldwide life expectancy increases, these diseases have become a tough challenge for healthcare systems as their prevalence is raising in the same way. The most important neurodegenerative disorders, because of their prevalence or their health impact, are Alzheimer´s, Parkinson´s and Huntington´s diseases; and amyotrophic lateral sclerosis. The knowledge about their origin is still very poor, so their clinical management is far from being adequate. There are not reliable methods for early diagnosis, so these diseases are usually detected when tissue damage is irreparable. Furthermore, current available therapies are intended only for symptomatic relief, since the misunderstanding about the real causes of these diseases and their delayed diagnosis hinder the development of new treatments being able to stop or even reverse the structural and functional decline. In the last few years, some pathological events have been identified as potential key factors to understand the origin of neurodegeneration. Among them, we shall mention oxidative stress, impaired proteostasis and neuroinflammation: a deeper knowledge about their interconnections and their ability to feedback each other could provide valuable information to progress towards an effective clinical management of neurodegenerative disorders.(AU)

Whole Person Care (WPC) is an emerging framework that emphasises the clinician's role in empowering patient healing. However, reliably translating a framework's theory into practice is a recognised challenge for clinicians. Observational studies have revealed discrepancies between a clinician's stated values in theory and how these may be implemented in practice. The aim of this qualitative study is to bridge the gap between the theory of WPC and its practical implementation by clinicians. We interviewed a diverse group of 34 clinicians attending the 2017 International Whole Person Care Congress to explore (1) their conceptions of WPC in theory as well as (2) how they monitor their practice in real time. Data were analysed using Grounded Theory Methodology. Preliminary results were presented in the form of a workshop at the 2019 International Whole Person Care Congress to validate our findings with relevant stakeholders. The results revealed a vision of WPC that highlighted themes of the clinician's way of being, seeing the person beyond the disease, and the clinician-patient relationship. Our results demonstrate that clinicians use a range of strategies to monitor their practice in real time. Mindfulness and self-awareness were frequently cited as being crucial to this ability of self-regulating their practice. This study helps establish a unifying framework of WPC based on a diverse range of clinician-reported experiences. More importantly, it sheds light on the range of strategies employed by clinicians who monitor their practice in real time. These collected insights will be of interest to any clinician interested in translating their stated values into their clinical practice more reliably.

Identity is a central aspect of our lives. Who we believe ourselves to be has profound implications on how we interact with and interpret the world around us. People with dementia often experience stigma and suboptimal care because of a perception that they have 'lost their identity' as the cognitive symptoms associated with the condition progress. This notion is challenged by proponents of person-centred care who argue that a person's identity and worth are based on more than cognitive abilities. Person-centred care is a holistic approach that takes account of the entirety of a person's life and provides support that respects the individual and their self-identity. Since identity is vital to well-being, delivery of person-centred care must involve actively seeking a person's concept of self to tailor their care to meet all their needs. This article considers the importance of identity and personhood for people with dementia and their carers in the context of person-centred care. The article includes two case studies to illustrate these concepts in practice.

Psychological presence in multidisciplinary obesity teams has been highlighted as an important component of such teams. Although mentioned in guidelines and recommendations, there is little information regarding the extent to which this is present currently in weight management services, and in what form. Here, we discuss important ways in which psychological aspects of obesity can impact a person living with obesity and how psychology can be incorporated to provide holistic support in weight management services. Recommendations are also made to create clearer guidelines to provide a more robust reference for the inclusion of psychology in multidisciplinary teams.
